Individual Problem Map

The Individual Problem Map is used to collect an initial decomposition of a problem, or Case according to a number of Problem Dimensions, along with rationale behind the relevance of this dimension in this instance and a relative ranking (high/medium/low) of the importance or criticality of the dimension.

As the name implies, the Individual Problem Map is completed individually by each member of the team (with the possible exception of the Process Coordinator).
